چکیده

Background and Aim: The clinical applicability of substitution of central venous oxygen saturation for mixed venous oxygen saturations in monitoring global tissue hypoxia is still a matter of controversy. Hence aim of the study is comparison of paired samples of mixed venous and central venous oxygen saturation and comparison in relation to cardiac index in varying hemodynamic conditio s. Materials and Methods: Prospective clinical observation: Postoperative cardiac surgical ITU: 60 adult patients, >18 years of age of either sex: A PAC was inserted through IJV, triple lumen catheter was inserted through IJV. Blood samples were taken from distal tip of PAC and central venous catheters. An arterial blood sample was drawn from either radial or femoral arterial line. Measurements: Continuous cardiac output monitoring. Analysis of blood samples for hemoglobin concentration and oxygen saturation. Mixed venous oxygen saturations and central venous oxygen saturations were compared. The study was carried over a period of 30h in the postoperative period and samples were taken at 6h intervals. Patients were classified into three groups as follows depending on the CI: Low (< 2.5 L/m), medium (2.5-4 L/m), high (> 4 L/ m2) and correlated with Svo 2 and Scvo 2 . Results: 298 Comparative sets of samples were obtained. Svo consistently lower than Scvo 2 throughout the study period. The difference was statistically significant. By using Bland Altman plot, the mean difference between Svo2 and Scvo2 (Svo2-Scvo2) was 2.9% ± 5.14 and confidence limits are + 7.17% and 12.97%. The co-efficient r is > 0.7 throughout the study period for all paired samples. The correlation Svo 2 and Scvo 2 with cardiac index in all the three groups were >0.7. Conclusion: Scvo2 and Svo2 are closely related and are interchangeable. Even though individual values differ trends in Scvo 2 may be substituted for trend in Svo 2

Influence of cardiac output on the correlation between mixed venous and central venous oxygen saturation.

The influence of cardiac output on the correlation between central venous oxygen saturation and mixed venous oxygen saturation was assessed in 51 patients who had both a pulmonary artery catheter and separate central venous catheter in situ. Seventy-six paired samples were taken from the catheters and oxygen saturation measured immediately in a Ciba Corning 2500 Co-oximeter. Cardiac output was ...

Critical Assessment of Use of Central Venous Oxygen Saturation as a Mirror of Mixed Venous Oxygen in Severely Ill Cardiac Patients By MELVIN

Bedside catheterization of the right heart with a percutaneously introduced flowdirected catheter was carried out in 24 critically ill patients; 71 determinations of mixed venous oxygen saturation (MVo2) were obtained. A second catheter was inserted either into a central intrathoracic vein or the right atrium for simultaneous sampling of central venous (CVo2) or right atrial (RAo2) oxygen satur...

Validity of Peripheral Capillary Oxygen Saturation and Normal Mixed Venous Oxygen Tension in Measurement of O2 Content

Background The change in venous oxygen saturation occurs earlier, and even its reduction is faster than arterial oxygen saturation. The aim of this study was to validate SvO2 and PvO2 for O2 content measurement in children hospitalized w...

Isoproterenol infusion and microcirculation in septic shock

INTRODUCTION Our study primarily aimed at investigating the effect of isoproterenol infusion on tissue oxygen saturation in patients with septic shock. The secondary aim was to assess the relation between cardiac index, central venous oxygen saturation and tissue oxygen saturation. METHODS This retrospective study was conducted from December 2010 to March 2012. We included 14 consecutive pati...

Mixed venous oxygen saturation in relation to cardiac output in myocardial infarction.

Mixed venous oxygen saturations derived from measurement of mixed venous oxygen tension were compared with dye dilution cardiac output determinations in 26 patients with acute myocardial infarction. Mixed venous oxygen saturation was greatly reduced in patients with shock or failure complicating myocardial infarction. The level of oxygen saturation correlated with cardiac output determinations....
